A Master Degree with a major or concentration in social work, rehabilitation, mental health counseling or another related field

AND

Experience providing case management services to mentally disabled (including individuals diagnosed with mental illness, mental retardation, alcoholism or substance abuse)

IN WHICH

two years of this experience included supervisory responsibilities for a program providing SCM or ICM Services.
