Busy Larchmont based Geriatric Care Management Group in business for over 15 years is seeking a Licensed Master of Social Work/Licensed Clinical Social Worker or Registered Nurse with geriatric experience. You would join a devoted team of Social Workers providing services to elder clients and their families in lower Westchester County.
Clients are seen in their homes, in our offices, in assisted living or nursing home facilities or hospitals. We collaborate closely with physicians, lawyers, clergy, and families to provide rapid response to crisis situations, to be strong advocates when needed, and to provide clinical comprehensive assessments to determine options for clients to get the best care available while making referrals for appropriate services (medical, psychiatric, hospice, home care staffing). Ongoing care management is provided during client home visits and contact with families by telephone or email.
Candidate must be able to work independently and also as part of a vital team. Must have strong oral and written skills, good interpersonal skills, strong organizational skills, and the ability to multi-task. Position requires that you are a driver with your own car available to use for local travel. Candidates must live locally within 10 miles of Larchmont, NY.
