The Field Nurse Supervisor RN is responsible for the direction, coordination, and direct supervision of the agency's paraprofessional staff in the performance of patient care. This position provides in-field professional supervision and on the job training to home health aides and personal care aides.

Job Responsibilities

Gives and interprets the patient information documents Provides patients/families with information regarding agency services, Bill of Rights, Advance Directives and agency policies Provides assessment of newly referred patients, assesses health needs and eligibility for home care Obtains patient's medical history as it relates to the patient's present condition Develops an individualized Plan of Care for the patient based on needs and findings of the nursing assessment Facilitates and documents patient's discharge from agency Performs annual and periodic field visits/evaluations on paraprofessional and nursing personne

Job Requirements

Active RN license in NY 1 year of nursing experience in a hospital or nursing home setting 1 year of home care experience strongly preferred Bilingual in English/Spanish required
